Job Title: Advanced Practice Clinician - Nurse Practitioner/Physician Assistant

Work Schedule: 40 hours/week

Duration: Approximately 7 - 8 months

Job Overview

The Senior Community Care (SCC) Program is a longitudinal, integrated care delivery program designed to coordinate the delivery of clinical care in a facility setting. The Advanced Practice Clinician (APC) within SCC provides care to our highest-risk health plan members as part of an interdisciplinary team.

Primary Responsibilities

• Conduct comprehensive assessments
• Manage acute and chronic medical and behavioral conditions effectively
• Ensure accurate and complete documentation, including ICD-10 conditions
• Communicate and collaborate with the interdisciplinary care team
• Facilitate advanced illness and advanced care planning discussions
• Provide patients and caregivers with counseling and education
  
  

Required Qualifications

• Active and unrestricted license in the state of residence or ability to obtain by start date
• Education:
• NP: Graduate of an accredited Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) program
• PA: Graduate of an accredited Physician Assistant (PA) degree program
• Certification:
• NP: Board-certified by the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC) with certification in one of the following specialties:
• Family Nurse Practitioner
• Adult Nurse Practitioner
• Gerontology Nurse Practitioner
• Adult-Gerontology Acute Care Nurse Practitioner
• PA: Board-certified by the National Commission on Certification of Physician Assistants (NCCPA)
• Current, active DEA licensure/prescriptive authority
• Ability to complete physical requirements of the role, including:
• Lifting a 30-pound bag
• Navigating stairs and various dwelling conditions
• Sitting, standing, and kneeling for physical assessments
• Reliable transportation
• Ability to gain a collaborative practice agreement, if applicable in the state
• Full COVID-19 vaccination required
  

Preferred Qualifications

• 1+ year of clinical experience (long-term care preferred)
• Experience managing complex behavioral, social, and functional needs
• Geriatrics & Chronic Illness expertise
• Knowledge of advanced illness and end-of-life care discussions
• Proficiency in electronic medical records (EMR) systems
  
  

Work Schedule & Travel

• Shift: 8 AM - 5 PM
• Days: Monday - Friday
• Dress Code: Medical attire
• Travel Locations: Osceola & Seminole County (During Shift)
• Facilities: 6-7
